1. Choose Your Registration Form. We offer two easy options:

  Complete the form electronically and save it to your device.

  Print the form, fill it out by hand, and scan or take a clear photo of each page.

2. Complete All Sections. Please make sure to provide:

• Child’s information

• Parent/guardian contact details

• Emergency contacts

• Medical information and permissions

• Any additional notes or special needs

• Add any dietary restriction and include if your child requires halal food option.

Incomplete forms may delay enrollment, so double-check all fields before submitting.

3. Submit Your Form. Once your form is complete:
